Do I need to include the date a property was remitted to the State Controller's Office to submit a claim?
No. This information is not required to submit a claim on behalf of any prospective client. However, this information is included in the Investigator DVD.
Does the Claim Affirmation Form need to be notarized?
Maybe.
- Claims for amounts $1000.00 or greater must be notarized.
- All claims for Securities or Safe Deposit Boxes must be notarized.
Can I notarize the Claim Affirmation Form for my client?
Yes. If the Investigator is also a notary then the Investigator is allowed to notarize the claim affirmation form for their client.
